About the DAISY Award?
What:
The DAISY (Disease Attacking the Immune System) Award us an international recognition program that honors and celebrates the skillful and compassionate care nurses provide every day. Active in thousands of healthcare facilities, around the world.
Why:
Established by the family of J. Patrick Barnes who died from complications of ITP in 1999. During his hospitalization, they deeply appreciated not only the immense clinical skill, but also the enormous compassion shown to Pat and his family by his nurses. When Pat died, they felt compelled to say “thank you!” to nurses in a very personal way.
How:
Honorees are nominated by patients, families, colleagues, and chosen by a committee of peers.
Impact:
Research supports the powerful impact of meaningful recognition and the Daisy Award.

How to Nominate a Nurse
Honorees are nominated by patients, families, colleagues, and with use of a criteria, chosen by peers.
There are 3 methods to submit a nomination form:
- To access an electronic nomination form or a PDF (print file), go to http://www.daisynomination.org/OCH
- Scan the QR code located on DAISY posters in elevators, hospital entrances, or patient rooms. See below:
- Download the nomination form (below) and email to [email protected] or send via mail.
- While in the hospital building, go to the nearest nurses’ station and ask about how to recognize and nominate your nurse for the DAISY Award.
